NZ resident, 18 or over
You are a New Zealand citizen or permanent resident, aged 18 or older.
Regular income
You have a regular income we can assess for affordability under the CCCFA.
Fertility treatment planned
You are planning IVF, ICSI, egg or embryo freezing, a transfer, a donor programme, or fertility investigations.
Able to provide ID and details
You can verify your identity and share basic income and treatment details.
